Medical and Family Leave
Employees who need time off for a qualifying family or medical reason are required to notify the district and complete all appropriate paperwork, prior to the commencement of the leave.
Employee Responsibilities:
- Complete the Family and Medical Leave Request Form and submit it to Benefits a minimum of 60 days prior to the requested leave, or per your working agreement. Upon submission of this form, additional paperwork will be provided to you by Benefits.
- Notify your direct supervisor of your need for time off and discuss sub arrangements, if needed.
- Employee and/or the supervisor will then contact HR for assistance with sub needs after determining the needs of the classroom/building.

- Contact Benefits to discuss changes/additions to your policies during your leave. For birth/adoption, you have 30 days from the date of birth/adoption to add a child to your policy.

The above leaves will run concurrent.
