Medical and Family Leave

Employees who need time off for a qualifying family or medical reason are required to notify the district and complete all appropriate paperwork, prior to the commencement of the leave.

Employee Responsibilities:

  1. Complete the Family and Medical Leave Request Form and submit it to Human Resources a minimum of 60 days prior to the requested leave, or per your working agreement. Upon submission of this form, additional paperwork will be provided to you by HR.
  2. Notify your direct supervisor of your need for time off and discuss sub arrangements, if needed.
  3. Employee and/or the supervisor will then contact HR for assistance with sub needs after determining the needs of the classroom/building.

  4. Contact Benefits to discuss changes/additions to your policies during your leave. For birth/adoption, you have 30 days from the date of birth/adoption to add a child to your policy.

The above leaves will run concurrent.
